Medication Management
Medication Management refers to developing a safe, effective system for organising and identifying prescription and over-the-counter medications.
Health Care Informed (HCI) will work with your organisation to ensure that best practice is being followed. Keeping track of all of the medications currently in use by a patient is an important part of medication management, HCI will examine the best ways to ensure that patients/residents are complying with their medication regime, while you adhere to best practice in administration, incident management and training.
HCI also works with your pharmacist (in-house or community) to maximise involvement and team work. It is only through a strong multi-disciplinary team that an effective medication management system can be implemented.
